The Conference booking form is available as a pdf file but it is quite large and may take a while to download over slower internet connections.  You can download the pdf using the link at the bottom of this page.

Alternatively (and recommended) you can print the form out as a picture directly from your printer; to read and fill it out that way.  This is far quicker and saves on bandwidth.  To do this just 'right-click' on each image, in turn, below and choose 'Print picture' from the menu that will appear.
